Health and Lifestyle Check List

This list is for Juice Plus Distributors who want to put on a 
Healthy Night program in their home. 
Give this list to each participant when they arrive.

Check off each statement that applies to you

I wake up tired after a nights sleep.

I often say, "Don’t talk to me, I haven’t had my coffee yet, or, I can’t think until I’ve had my coffee."

I want to cut down or quit smoking.

I want to have more energy.

I want to cut down my alcohol consumption.

I eat fast food more than twice a week.

I am not as active as I want to be.

I want to lose weight.

I have tried different diets, but go back to my old habits.

I want to be in better shape.

I lack the time or motivation to exercise.

I could go a whole day without eating vegetables.

I could go a whole day without eating fruit.

I want to eat more raw fruits and veggies to meet the daily requirement of 5-9 servings per day.

I find moldy vegetables in my refrigerator and I end up throwing them out.

I am looking for an easier way to ensure I get all the fruits and vegetables I need.

I am addicted to sugar and sweets.

I spend over $40 a month on products that contain sugar and fat.

I find myself spending over $2 in coffee shops.

I haven’t really made my health a priority, and it shows.
